Indication
A medication called Telcardis 40 is used to treat heart disease and high blood pressure. Blood pressure reduction aids in reducing the risk of future heart attacks and strokes. Using this medication can help diabetic patients maintain kidney function. One may be prescribed Telcardis 40 alone or in conjunction with other drugs.
Pharmacology
Telmisartan selectively prevents angiotensin II from binding to the AT1 receptor in a variety of tissues, including vascular smooth muscle and the adrenal gland, in order to inhibit the vasoconstrictor and aldosterone-secreting effects of angiotensin II. As a result, the angiotensin II synthesis pathway has no bearing on its activity. When compared to the AT2 receptor, telmisartan's affinity for the AT1 receptor is >3000-fold higher. Telmisartan has no effect on the reaction to bradykinin because it does not inhibit ACE (kininase II). Other hormone receptors and ion channels known to play a role in cardiovascular regulation are not bound by or blocked by telmisartan.
Dosage & Administration
Individual dosage must be used. Telmisartan tablets are typically started at a dose of 40 mg once daily. Over the dosage range of 20 to 80 mg, the blood pressure response is dose-related. The maximum reduction is typically reached after 4 weeks, with the majority of the antihypertensive effect becoming apparent after 2 weeks. You can switch to the combination when additional blood pressure lowering beyond what is achieved with 80 mg Telmisartan is needed. Elderly patients and patients with renal impairment, including those receiving hemodialysis, do not require an initial dosage adjustment. Orthostatic hypotension can develop in dialysis patients, so it's important to closely monitor their blood pressure. Other antihypertensive medications may be given along with telmisartan tablets. You can take telmisartan tablets with or without food. In patients under 75 years of age or with hepatic impairment, initial therapy with telmisartan is not advised.
Interactions
NSAIDs: Increase the risk of renal failure and loss of antihypertensive effects.
Combination of Aliskiren and Telmisartan: It should be avoided in diabetic patients.
Contraindications
Known allergy to this product or any of its ingredients.
Side Effects
Pregnancy
Pregnancy category C (first trimester) and D (second and third trimester). Due to the potential for adverse effects in the nursing infant, a decision should be made to either discontinue breastfeeding or discontinue the drug, taking into account the importance of the drug to the mother.
